Routes Of Referral


There are a number of ways to get referred to the scheme.

The most common way of being referred to us is by the Community Mental Health Team (CMHT) here at the Hawthorn Centre. This team includes Psychiatrists, Community Psychiatric Nurses (CPNs), Social Workers and Support Workers.

Referrals are also sometimes made to us by GPs and we can take self-referrals.

Once you have been referred to us, Sandra, the Scheme Manager will contact you to arrange a convenient time to visit you in your own home. Sandra will discuss with you how the scheme can best help you and which of our services is best suited to your needs.
